Fees: 
■ Eye Examinations for $30  (incl. $5.00 Clinic Fee)  
 
What do you have to do? 
 
  Sign up at Shifa Clinic, Sugar Land on any Saturday (during clinic hours) 
prior to SATURDAY, OCTOBER 11, 2014
  Appointments must be made in person and not by phone or email. 
  Sign up sheet is available at the reception desk.  
  There is a $10 NON-REFUNDABLE deposit (CASH ONLY) required.  
  Remainder $ 20 (CASH ONLY) due on the day of examination 
  We only have room for 15 patients. Please do not wait until the last minute. 
  APPOINTMENTS GIVEN ON FIRST COME FIRST SERVED BASIS. 
  NO CONSULTING WITHOUT PRIOR APPOINTMENT….NO EXCEPTIONS!!!!!!!!! 
The Mobile Eye Institute offers: 
  Comprehensive eye examinations by APPOINTMENTS ONLY. 
  MEI optometrists will also diagnose common eye diseases such as allergies, 
dry eye or conjunctivitis.  
  Prescriptions for eyeglasses (NOT CONTACT LENS!) will be written as well as 
appropriate referrals for specialized vision care.
