prior to SATURDAY, OCTOBER 11, 2014
Appointments must be made in person and not by phone or email.
Sign up sheet is available at the reception desk.
There is a $10 NON-REFUNDABLE deposit (CASH ONLY) required.
Remainder $ 20 (CASH ONLY) due on the day of examination
We only have room for 15 patients. Please do not wait until the last minute.

The Mobile Eye Institute offers:
Comprehensive eye examinations by APPOINTMENTS ONLY.
MEI optometrists will also diagnose common eye diseases such as allergies,
dry eye or conjunctivitis.
Prescriptions for eyeglasses (NOT CONTACT LENS!) will be written as well as
appropriate referrals for specialized vision care.
